Your Appointment
We see all patients (except emergencies) on an appointment basis so that we can reserve an adequate amount of time to provide the dental care you require. We make a sincere effort to honor all time commitments, as we know your time is valuable. We request that you extend the same courtesy to us.
To Make An Appointment
Please call and tell our receptionist why you wish to see the doctor. It will help her schedule the proper amount of time and appropriate treatment for your appointment.
To Cancel An Appointment:
Please give us 48 hours notice. This advance notice allow us to schedule another patient in need. We will reschedule your appointment for a more convenient time. There is a $25.00 cancellation fee for unexcused cancellations made within 48 hours of appointment.
To See A Doctor Without An Appointment:
Call our office and explain the problem. We will help all we can, as quickly as we can with proper respect, of course, for our patients with previously scheduled appointments.
Skipping An Appointment Without Calling:
This is unfair to the doctor our staff and other patients waiting for dental care. Sure, emergencies arise and a scheduled appointment is sometimes overlooked. However, when someone consistently skips appointments, we assume they are not serious about their dental care – and we are. PLEASE keep your scheduled appointment.
Patient Forms
If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Thank you and please contact our office if you have any questions at all.
Fees And Payment Policy
In an effort to keep dental costs down, while maintaining a high level of professional care we have established the following payment plans for the use of our patients. Fees may be paid as follows:
1. Prepayment of charges at a time of treatment with a 5% discount.
2. Payment of balance in full upon receipt of statement.
3. Use of your Visa or MasterCard.
Extended payments are available upon special request. Accounts outstanding more than 30 days from treatment date will bear interest at 1 1/2 % per month or 18% per annum.
All treatment involving a laboratory procedure will require an appropriate down payment.
Our senior citizens will receive a 10% discount if payment is made at time of a treatment.
Dental Insurance
Many of our patients have dental insurance or are receiving dental insurance benefits for the first time. It is important to remember your dental insurance policy is an agreement between you and the insurance company. We are only a third party to this agreement. We will help make this agreement work for you by providing all the information you need to receive prompt payment from your insurance company. You are responsible for payment to this office at the time dental services are rendered.
Please remember that no insurance plan covers all dental costs. Some companies pay fixed allowances for certain procedures and other pay a percentage of the charge.
We will be glad to help you complete your insurance forms.
